
RIGHT TO REQUEST A FORM 1095-C
In accordance with federal law, the City of Springfield will no longer automatically mail the Form 1095-C, beginning with the 2025 tax year. Current and former employees still have the right to request their Form 1095-C once the form is available.
To request a printed copy of your Form 1095-C to be mailed to your home address, contact apply@springfieldohio.gov or mail the request to the City of Springfield Human Resources Department at 76 E. High Street, Springfield, Ohio 45502. Please call 937-324-7321 if you have any questions.
The City of Springfield provides employees with a tax form called a 1095-C as part of our reporting requirements under the Affordable Care Act (ACA). The 1095-C contains important information about medical coverage offered to you and your dependents by the City of Springfield.
Recently two acts, the Employer Reporting Improvement Act and the Paperwork Burden Reduction Act, introduced several changes to the reporting and enforcement rules of the ACA. The two acts no longer require employers to send the Form 1095-C to all full-time employees and covered individuals; however, as an employee you have a right to request a Form 1095-C.
